Fixed 9255's Activity defects: /fail after an answered blocker round is now terminal, and the vanished note no longer repeats the prior attempt's reason. Quick gate + tests green; PR #6716 merged; dev apply OK (platform-bundles hit a GitHub timeout). Stopped 9255; draft 9267 filed.
