Audit and Support screens polish fixes

Seen while checking Task 8565's screens on a local stack at 390 px and 1440 px. 1. Audit › Activity: a sign-in refused for an unknown address shows "Operator" in the folded line's Org column, but its detail's Account reads "Unattributed". Show the same word in both places. 2. Audit › Sessions: the "Attempts with no session" block has a NODE column header at the far right with no values under it at 1440 px. Either fill the column or drop the header. 3. Audit › Sessions › Efficiency: at 1440 px the unconfigured line wraps so that ".env" sits alone on the next line. Keep "the stack's .env" together. 4. Support › All: operators can see "Lesson to approve" on a row but cannot filter by it. Add a Lesson filter, or a count on the tab, so a backlog of drafts can be found. 5. Compliance matrix: an errored row says "Ask an editor to rescan" next to the error. Check that this wording is right for a viewer who can rescan the row, who should get the Rescan button instead. Out of scope: scan engine and packs, refusal wording, session data and permissions. Done means: - Each item is fixed or rejected, with a one-line reason in the PR. - A screenshot of each changed surface at 390 px and 1440 px.

Fixed 4 of 5: Activity fold Org = Unattributed, empty Node heading dropped, ".env" kept together, Support All Lesson filter (operator-only API param). Item 5 rejected (wording already correct). Quick gate + tests green; PR #6506 queued. Follow-up draft #8651. Memory: none applied.
