Outcome

Jaah bug, not Codex: the mapper dropped Codex's question when its label was over 12 chars. Fixed via AcpDialect.question_meta_key (label shown in full) plus a regression test; quick gate green; PR #7330 queued to the merge lane. No UI surface affected. Memory: none applied.
